This book was inspired by more than a decade of hands-on experience in cybersecurity, governance, risk, and compliance. Throughout my professional journey, I have observed that the greatest threats to organizations are not always external — they are often internal, gradual, and silent. The motivation behind this work was to confront those invisible failures. The models introduced in this book — the Privilege Maturity Index (PMI), Control Drift Taxonomy (CDT), and Access Governance Risk Score (AGRS) — are the outcome of real observations and professional insights. They are designed to help organizations identify, quantify, and correct governance drift before it turns into a crisis.
